Thank you for considering Donna’s Kidz Sunflower Academy for your family. We know choosing childcare is an important decision, and we’re honored to be considered as a place for your child to learn, play, and grow.

To help make enrollment as smooth as possible, families will need to complete the required enrollment paperwork and submit all applicable fees before their child’s first day of care.

Our programs serve children from 6 weeks through 12 years old, including full-time childcare, school-age care, drop-in care based on availability, and summer programming.

Before your child can begin attending Donna’s Kidz Sunflower Academy, all required enrollment paperwork must be completed and submitted.

Families will need:

• Completed Enrollment Application 
• Copy of the child’s birth certificate 
• Current immunization records or approved exemption documentation as permitted by state regulations 
• Emergency Contact Information Form 
• Medical Information Form 
• Authorized Pick-Up List 
• Signed Parent Handbook Acknowledgment Form  

Parents and guardians are responsible for keeping emergency contacts, medical information, authorized pickup individuals, and other enrollment information current. 

A $75 non-refundable registration fee per child is required at the time of enrollment to reserve your child’s space. Families must also pay the first week’s tuition and last week’s tuition before their child begins attending.

The last week’s tuition will be held on the family’s account and applied to the child’s final week of care, provided proper notice of withdrawal is given according to academy policies.

Enrollment is not considered complete until the required enrollment fees and tuition deposits have been received. 

Once all required enrollment paperwork, fees, and tuition deposits have been completed, your family will be ready to begin your journey with Donna’s Kidz Sunflower Academy. We look forward to partnering with you and providing a safe, nurturing environment where your child can learn, grow, and shine.
